Une antibioprophylaxie chirurgicale est recommandée

lors de procédures chirurgicales associées à des taux élevés d’ISO (procédures propres-contaminées ou contaminées), ainsi que
lors de procédures pour lesquelles les conséquences d’une infection sont sévères (p.ex. implants prothétiques), même si le risque d’ISO est très faible.

Une antibioprophylaxie chirurgicale n’est pas recommandée dans certaines procédures propres, même si le risque relatif de réduction des ISO lié à l’antibioprophylaxie est le même pour les procédures propres que pour celles à plus haut risque. Les coûts induits par une ISO (traitement et morbidité associée) comparativement aux coûts induits par l’antibioprophylaxie et la morbidité associée sont pris en compte dans la décision de recommander ou non une antibioprophylaxie.

L’utilisation d’antibiotiques lors de procédures sales ou d’infections avérées est considérée comme le traitement d’une infection et non pas une prophylaxie.

Nous avons adopté une limite générale de poids de 80kg pour augmenter la dose (Ann Surg. 2011;253(6):1082-93). Les recommandations swissnoso proposent 80kg ou 120kg, selon la substance.

Remarque :	Patients à haut risque: Une prophylaxie doit être envisagée chez les patients à plus haut risque d'infections gastroduodénales postopératoires, en particulier chez les patients avec pH gastrique augmenté (p.ex. les patients sous antagonistes des récepteurs de l'histamine – anti H2 ou sous inhibiteurs de la pompe à protons), perforation gastro-duodénale, diminution de la motilité gastrique, obstruction de la vidange gastrique, saignement gastrique, obésité morbide, cancer.
